You can find a plethora of online forums that can help you with survey questionnaires. Google Forms is the one you might find very useful. You can create a form and share it with groups on social media or here as well.
In addition to this, you can also create a simple HTML based webpage and share the link with people.
Agree with Asif, google forms is the best when it comes to data collection. Remember the way data will be analyzed and appropriately choose the right settings during data collection to make life easier.
Thanks for your answers! Well, I have already tried Google Forums multiple times and based on our experience, it has some error on collecting data, some are counted twice or thrice and some are not counted. Any other recommendations other than Google Forums?

James Aaron Erang, maybe check out SurveyMonkey or Qualtrics. In my experience, they are both sophisticated. I don't know how much they would charge for the kind of research you want to do, but maybe you are at an institution that has already paid for a licence to use them.
