The Resnick and Niwayama criteria to diagnose Diffuse Idiopathic Skeletal Hyperostosis (DISH) include the preservation of intervertebral disc height in the involved areas without degenerative disc disease (2nd criterion). 

Nevertheless, it is well known that primary OA and DISH may coexist. In this way, it is possible to find an ossification of the anterior longitudinal ligament of at least 4 contiguous vertebral bodies with the presence of a narrowing of the intervertebral disc spaces (see the figure)

If it is so, what about Resnick and Niwayama’s second criterion? Should we discard it? Or by contrast, must we classify this image as DISH (-)?

I'd like to know your suggestions.
