A combination of dobutamine with noradrenaline is reasonable and safe as long as blood pressure is not the only target (others are normalisation of acid-base status, increase in urine output or actual measurement of cardiac output. A lot of clicians use adrenaline; in my opinion adrenaline infusions are responsible for a lot of hyperlactataemia we see in septic patients! (my opinion entirely). There is a lot more to patients than just raised blood pressure. 

Choice of vasopressors depends mainly on the underlaying condition wich we are treating. Preference of certain drug does have nothing to do with it. Severe metabolic acidosis  is offten responsibile  for increasing doses of vasopressors that, as we know it, act only in a certain pH range  and the first condition needed for these drugs to act is sometimes to correct the acidosis first. We sometimes do use the epinephrine/norepinephrine combination  when we need to increase peripheral afterload like in sepsis. adding the epinephrine  to norepinephrine we get vasoconstriction with smaller doses of norepinephrine and we also get positive beta effects of epinephrine ( positive because in sepsis we often have septic cardiomiopathy present and some degree of positive inotropic action is needed). Our choice of vasoactive drugs is mostly based on invasive monitoring rests and addopted to each patient individually, with no generalistion

the recommended choice is first noradrenaline associated with dobutamine in the presence of heart failure. A place for vasopressin or terlipressin (in France) is also proposed. the alternative with the adrenaline still debated

Thanks a lot for your kind responses, I do agree with combination of noradrenaline with dobutamine, sometimes I administer a combined of noradrenaline with nitroglycerine 

You do not have right to treat these patients. This situation cannot be solved by the answer to a multiple showiest question. How did you get into that situation
