Dear David, I think your question is not clear. Instrument(Questionnaire) to assess the impact of health education depends upon the topic to be taken into consideration. But according to my view, we should follow the following steps:For this first we select the problem. Then to know the level of awareness we must conduct the baseline survey regarding the selected problem. Second is to what you expect the outcome after intervention(health education) must clear to us, then we plan the intervention, for this we should have teams which are for different activities, after designation of  the target population, preparation of message to be delivered through specific media, then team member for dissemination of message, and also supervisory team for monitoring the activities , then statistical team to analyse the data and expert of the subject to interpret any improvement in the level of awareness among the designated population after health education.

Can you please msg me as I am interested in doing a study on the long term effects of early childhood nutrition education on health outcomes. 

In regards to your question I suggest you use a questionnaire for your study population. I am sure that you know about these already but I will mention them anyway, there is the REALM which I don't like because it does not test comprehension and the Newest Vital Sign (NVS). Also there is the Test of Functional Health Literacy in Adults (TOFHLA) that I find is pretty comprehensive, and also the Health Literacy Questionnaire (HLQ) that I also like. 

I can suggest 2 review articles on health literacy assessment tools:

1) Critical appraisal of health literacy indices revealed variable underlying constructs, narrow content and psychometric weaknesses. Jordan, Joanne E. et al. Journal of Clinical Epidemiology , Volume 64 , Issue 4 , 366 - 379

2) Health Literacy Measurement: An Inventory and Descriptive Summary of 51 Instruments. Journal of Health Communication: International Perspectives, Volume 19, Supplement 2, 2014

these articles may help you determine which one is best for you study population.
