I am looking at the psychological and health impact of experiencing sudden and unexpected bereavement. I intend to use the IES-R, HAD and inventory of complicated grief but want to look at how their experience has also affected their general health.
I recommend the SF 36 questionnaire. It can be found on the internet, I think it can meet your needs.

I have used the SF 36. It has good psychometric data, has been used in numerous studies and addresses various aspects of health--physical, psychological, social.
If you are looking for something briefer, you could use the abbreviated SF-12 or SF-8, which also have good psychometric validation. These are particularly useful in settings where long questionnaires are difficult to implement (e.g., hospital settings).
Thank you for your suggestions. I think a shorted version (SF-8) may be the most appropriate as there will be a number of other measures included.
