Can someone recommend a good polymer book for someone whose background is not in polymers, but has a general understanding of how they work and sufficient background in materials science? I am mainly looking for a book that is application-focused, with a lot of real world examples of different common polymers for different applications. Something not too fundamental and hopefully easy to read.
As a good introductory textbook, which, covers a broad range of topics (to different degrees of detail), is concisely written and very enjoyable to read, is:

Other than that I also enjoy Rubinstein's ,Gedde's and Painter and Coleman's classical textbooks. If you are well versed in material science, I think, it might also be an idea to get a "Handbook", as a collection of different polymer properties. They often have an introductory section which explains the underlying polymer physical concept. So e.g.: Physical Properties of Polymers Handbook by James E. Mark or Polymer Handbook by J. Brandrup et. al. feature such introductory sections. Also, in general I can highly highly recommend to get a version of Flory's Principles of Polymer Chemistry, which is mainly dealing with Physical Chemistry though. If you tell me a specific subject you are interesting in I can probably make a better educated guess of what might suit your needs. Examples could be textbooks on the topic of crystalline polymers or on (block)copolymers.

If you want a general background on polymer physics (how do polymer chains move and how does this relate to the properties we experience everyday), then I highly recommend Masao Doi's "Introduction to Polymer Physics". The book is real short and explains the basic thermodynamics and derivations for polymer motion in an extremely clear manner.
Rubinstein's textbook is another great introduction to polymer physics (especially scaling principles, percolation, and fractal behavior), and Hiemenez and Lodge's book is a good resource for polymer chemistry. The Hiemenez textbook also has a good perspective on applications (from what I remember).
I can totally double the recommendation for Hiemenz, but I think that might not be exactly to the point for what you are looking for. If you decide to go for Doi do not confuse his introductory textbook "Introduction to Polymer Physics" with his reference textbook on polymer dynamics. In addition Doi wrote a new book called soft matter physics.

I taught polymer chemistry for more than 25 years & I appreciate that it is not easy to find what you are looking for. However, I recieved a very good book as a gift in 1994:
Ian M. Campbell, Introduction to Synthetic Polymers,Oxford University Press, Oxford, U.K (1994).
Professor Campbell is a distinguished professor who used to work in the University of Leeds, England (U.K). I wish you good luck in finding it or the newer edition of it (published in the year 2000, but I did not see it).
