OK, basically most people with this condition aren't in treatment or seeking treatment, therefore population health approaches are needed. Funding better treatment services is unlikely to make much difference.

Control of price, taxation, availability & advertising, along with police targeting drink driving is what is required.

The first issue is whether the individual manifests a loss of control over drinking. With the DSM-IV a dependence diagnosis could be given for spending a great deal of time drinking, occasionally drinking more than intended, and developing tolerance from the amount consumed. In this example, there is really no indication that the individual could not moderate use. On the other hand you could have someone who has clear indications of loss of control as indicated by an inability to limit use, has intense cravings or compulsion to use, etc.

We have published a number of articles on alcohol, cannabis, and cocaine that indicate that a subset of the DSM-5 criteria when positive are most likely to be found among those with a severe diagnosis. These criteria would appear to be compatible with the construct of loss of control, and almost all who reach a severe diagnosis of the DSM-5 are likely to be positive for at least some of these criteria.
