Do you mean in nursing education or among nurses. I have the evidence-based education questionnaire (EBEQ) which has been validated in the us and elsewhere and is suitable for both nurse educators and clinical instructors. It asks questions about factors influencing the adoption of EBP in nursing UG education and the other tool addresses these factors that influence nurses' adoption of EBP in nursing practice. Hope this helps.
Sure. I own the rights for this tool. You have it attached in-full. If you will be using it, then kindly write the following citation:

Carefully with evidence based concept. This is a statistical equivalent of a large material. Individual patients need individualized, patient-specific treatment and care. Validating questionnaires is a technical issue.
I would surely advocate the response of Ivan. But still we need to understand that pathophysiology is almost similar in different human beings, which makes it possible for us to advocate EBP dealing with apparently comparable situations. In addition, in the case of our tool, we are dealing with a research instrument that addresses factors that cause the adoption of EBP is challenge. We need to agree on the minimal in order to decrease discrepancies and maximize benefits.
