In tooth bearing areas, non expanded lesions, to make a clinician confirm a diagnosis of KOTs before a definitive treatment has been done. there are certain lesion have some clinical, radiographical features and preoperative diagnosis is very important because this lesions have a different treatments, thus, could a CT dose differentiate between keratocystic odontogenic tumors and other cystic lesions?

I think CT Scans are mandatory in the diagnosis of large cystic lesions to clarify differences between KOTs and other cystic lesions. But I think the only method to establish the final diagnosis is doing a biopsy followed by an anatomopathological exam and to define the therapeutic assesment.

although Biopsy is still the Gold standard, MS CT scan can help in identifying the extent of the tumour. CT scan is superior to CBCT till date.
The sensitivity and specificity of CT for distinguishing OKC from other cystic lesions is not great. Sometimes they have a certain texture that may help, but many don't and at the end of the day, tissue diagnosis will be way to confirm.
No CT is not going to be any more effective than a regular radiograph, in differentiating KCOT from other cyctic lesions, unless the other lesions have anything specific.
