In your survey you should ask the following questions :

Are you satisfied with the thermal comfort? yes no

How is your thermal feeling?

-3 Too cold -2 cold -1 fresh 0 neutral 1 warm 2 hot 3 too hot

This provides you the basic data. The you can calculate the percentage of dissatisfied form the answers to the first question and the mean vote from the answers to the second question.
