The mechanisms of biological evolution, principally adaptive learning responses as response to external selection pressure, are very badly understood today. This is a broad and important question. The shortage of drug supplies during the pandemic stemmed from disruptions in the availability of raw materials required for manufacturing, impeded transportation from factories to government distribution agencies, and delayed allocation of limited resources to primary health centers and hospitals nationwide. These interruptions led to delays in initiating ATT, temporary treatment discontinuation in some patients, and permanent suspension in others. Collectively, such disruptions increased the incidence and prevalence of tuberculosis, thereby contributing to the global rise of MDR and XDR strains.

Darwin’s theory of natural selection, which operates at both inter- and intra-species levels, remains highly relevant in this context. Nevertheless, governments tend to prioritize pathogens that pose immediate and large-scale threats. The COVID-19 pandemic rapidly disrupted nearly all forms of production and social functioning, whereas tuberculosis and other chronic diseases have not crippled the world with comparable speed. Indeed, no generation in the past century—approximately 2.5 to 3 human generations—had experienced a global epidemic of such scale. The WHO has repeatedly emphasized the need to maintain focus on tuberculosis and other diseases alongside COVID-19. The fundamental challenge for every country, however, lies in allocating scarce resources across competing health priorities, a task that can only be addressed through strategic prioritization.
