Hi Arti, Duchefa [https://www.duchefa-biochemie.com/] sells many components for and ready made media for plant tissue culture. Their documentation should give you a good overview for nutrient concentrations etc. Regarding phytohormones, this will be different from application to application and from species to species. I would advice to ggogle for your specific case, you should be able to find some protocol.
The application rates of plant hormones depends on various factors, such as the type of hormone, plant species, stage of plant development, method of application. Generally speaking, auxins such as IAA is applied at the rate of 0.1---10 mg-l. NAA up to25 mg-l. gibberellins could be applied up to 100mg-l, cytokinins up to 200mg-l. if you are working on tissue culture very much lower concentrations of plant hormones should be used.
