I was not aware that paroxetine loses efficacy over time. Can you reference the source of this information or otherwise elaborate if it's anectodal.
Actually, I believe the current understanding is that SSRI's require several weeks to have a therapeutic effect.
MAOI's may possibly interact with dietary tryptophan (serotonin precursor) resulting in the potential of serotonin syndrome, though it seems the risk is with pharmacological rather than food sources. I don't believe there are any such issues with SSRI's like paroxetine. See prescribing information for these kinds of details.
i am using paroxetine associated with bromazepam since year 2000 to treat my panic desorder and agoraphobia, it worked good in the first 7 years, but now and since year 2008 i feel that the medications dont gives the primary effects, and the symptoms becomes worst, i know that bromazepam causes downregulation of GABAa receptors, i want to know if samely there is serotonin transporter downregulation as well?
Since SSRI's bind to and inhibit the action of SERT (serotonin transporter), the downregulation of SERT should not theoretically attenuate SSRI efficacy. Actually there is some evidence that SSRI's exert their therapeutic, or at least their direct functional effect, via SERT downregulation. A large number of receptors and neurochemical systems are affected by the chronic elevation of extracellular serotonin levels. As such, antidepressant tachyphylaxis (loss of efficacy over time) is a complex question, though a phenomenon that is unfortunately quite common and not unique to paroxetine (see Targum, 2014).
