Why does the active fiber break in the middle?

I have a simple oscillator whose output wavelength is 1018 nm. In all 3 attempts, the active fiber broke within 60-80 cm of the HR FBG. What is the cause and which nonlinear phenomenon do you think could be the cause of physical damage to the active fiber?

I think, Thermal lensing + photodarkening is the dominant issue. Have you checked the effect of pump power sensitivity?
