Netsim 10 is the best for simulate cognitive radio networks but you need license copy also I am not sure but heard NS-2 is good.
We have used NetSim in the past. It is kinda limited when it comes to simulating CRNs but no other simulator has a built-in dedicated CRN module. Extending the module as per your requirements is also possible. Codes are in C++. Hope this helps.

As Amar mentioned source C codes are available which can be modified.
But first you need to decide what is it you actually want to simulate in CRN?
