Which SD should I use in calculating the percentage of patients with one SD decline in cognitive functioning?

Dear all,

I am currently conducting an analysis on longitudinal data. I would like to determine the percentage of patients with at least 1 SD decline in neurocognitive functioning. I test the patients 4 times: pre-treatment, 3, 6, and 12 months post-treatment. Due to treatment, it is expected that the patients perform worse post-treatment.

Which SD should I use? The SD of all testings? Of only the SD at baseline?
