It depends on what you're trying to measure. If you are looking at risk assessment, then I would suggest the ERASOR (Worling & Curwen) or the J-SOAP (Prentky & Righthand). Otherwise, there are a number of good book written about juveniles and risky sexual behaviour (try Phil Rich, David Prescott, or a new book by a number of authors including Rob Longo, David Prescott and others). However, all of this is going to pertain mostly to youth who have engaged in sexually offensive behaviour. For other risky sexual behaviours, I am not quite as familiar with that literature, but I bet that any of the folks named above (if they are RG members) would be happy to comment.

You might look at the Sexual Experiences Survey by Mary Koss and her colleagues. I use the version from 1982, which I've found to be still relevant today with strong psychometrics. There is an updated version that assesses the experience of sexual violence more broadly that Dr. Koss put together a few years ago. It was more extensive than I needed, but could suit your purposes.
Denise see if this document helps. It is not a questionnaire itself, but assessing these issues was what Douglas Kirby (the author) spent his life doing. On pages 49, 67, 81, 89, and 102 there are questions which can be used to create such a questionnaire.
Additionally, here are some good articles to get: Adult Attachment and Sexual Behavior by Bogaert and Sadava; Attachment, sexual experience, and sexual pressure in romantic relationships: A dyadic approach, by Brassard, Shaver, and Lucier; Associations between insecure attachment and sexual experiences, by Gentzler and Kerns. Also, I would recommend the book (somewhat dated) Attachment Theory and Close Relationships, edited by Rholes and Simpson.
