I think you can use a general physical assessment to measure risk on teleworking/computer operator posture. You can try the Quick Exposure Checklist (QEC).
If you start from scratch, you can use the Nordic body map to identify the problem and then analyze the posture. To measure the risk, you can use Rapid Upper Limb Assessment (RULA) because it is a sitting working position. After that, you can redesign the facilities according to the result.
I agree with the Andrijanto Andrijanto statement. Furthermore, we can be focused on the assessment of facilities including technical aids and the practices adopted for access. Also, adjustment to the job and the posture.
Your question has some parts. Firstly, you want to evaluate physical stress and for that Nordic Questionnaire or Body Map Scale may use. Secondly, your survey on computer workers so psychological assessment like MoCA, Mini mental State etc. may useful. And for posture evaluation REBA or RULA useful.
