Characteristics of the Nursing leadership influence growth and development of the nursing profession.
I am very interested in up-bringing the excellence, Quality of Nursing. Autonomy lies and leads to Quality of Nursing.
Nursing should govern by autonomy. Autonomy lets and leads to establish the lines for Quality of Nursing.
Japanese nurses are disempowermented by Medical Act, which prohibits any medical procedures without doctor's involvement. In my opinion, doctors' direct/indirect supervision in the community setting is unrealistic. Nurses should have more responsibility. I think it is time to decide what kind of responsibility is required for community nurses. Then, we can apply some result from that discussion to hospital settings. Also, nursing department should realise the trend to catch up with the requirement for new generation.

Good quality in nursing can only occur if proper training, education and research are in place. The question is tricky though, as not much studies seem to be available that explore the effects of poor nursing on training and research. Moreover, poor performance might not be detected as such and therefore have some unnoticed effects as well...
Thank you Ralf for the references.Lack of autonomy for professional education,training and research (Unlike Nursing service side) often results into a poor performance review process. Quality issues going un noticed ultimaletly affect it's real benefeciaries i.e. the RNs and patients.
The nursing leadership plays a fundamental role in the development of the nursing profession at all levels, national and local levels. In Italy, only recently a parliamentary law has established the professional autonomy (1999) and provided for the nursing leadership in any organization of the National Health Service. Since 2005, several Italian universities have started master degree strongly oriented to the training of nurse managers.
As nursing represents care,Managerial role represents areas beyond the scope of care ie.HR, finance,budget, materials management , business management ,quality,customer service,policy decisions.Therefore in addition to Batchelor's degree in nursing (4 years duration in India )for an admin role,MBA has the most appropriate curricula. Masters in nursing is aimed at preparing nurses role in the areas of Nursing education,clinical speciality ,clinical practice,nursing research, evidence based practice and patient education at advanced level. Compared to MBA the MSN curricula has limited exposure for a Director's or Managerial role.Thus to practice autonomy for the benefit of health care industry and patient population, nurses with appropriate education and experience are required to lead specific areas.
I think We need to appreciate the objectives of two different programmes and the clientele. Care is implemented with support of all the areas focused there in masters nursing curriculum The difference is that how the teachers and administrators handle the curriculum but that is not enough The focus changes with time and experience that is why life long learning depending upon the requirement of individual client should be pursued as a remedial measure for fulfilling organization goals

Talking about autonomy of nurses is an old as well as ever new topic. In my opinion autonomy is not given but instead it is gained. Proving to hospital managers and other health care personnel that you are competent and are doing your job in the best possible way is important to gain confidence and independence. In addition nurse managers and leaders either facilitate or inhibit the autonomy of nurses. Being assertive and defending the nurses' rights and empowering nurses are important to gain autonomy.
Autonomy calls for competence, advanced knowledge and skills, lifelong learning, research and evidence based practice, decision making skills, communication skills, and collaborative skills to provide quality care.
