Anterior traumatic shoulder dislocation is commonly seen injury in young and active individuals. When should one ask for MRI or CT scan of shoulder in First time shoulder dislocation ?

In a first time scenery dislocation is better to study first with XR and MRI. The MRI will give you more information about the soft tissue lesion like Bankart lesion, HAGL, Rotator cuff, etc. After the XR and MRI if you need more details about humeral or Glenoid bone defect I will recommend to ask for a 3D CT scan with Glenoid face view to determine the Glenoid track or to look for an acute bonny Bankart
MRI- shows the labrum lesion very well but also gives an estimate of the size and location of the Hill-Sachs lesion and anterior bone loss.
A CT scan is only necessary for
a) absolute quantification of bone loss/Hill Sachs
b) to create a 3-D model of a Bankart fracture
the initial X-rays will guide the surgeon if an additional CT is indicated.
