My sample size is 18 including patient and control . I have used target gene and reference gene .
Many replays are recommended the delta ct value, but still unclear why i should use it ? what about the delta delta ct ? and according to my sample size what is the best test ( t-test or Mann-Whitney)
The dCt is what you measure, as an indicator of the expression of your gene of interest. If you really don't know why you should use it, I wonder why you do qPCR...
The t-test tests the null hypothesis that the mean difference is zero. The Mann-Whitney test tests the null hypothesis that the samples are stochastically equivalent. So to answer the question which test to use is to answer what hypothesis you want to test.
First of all, you should check normality then you can decide using t-test or Mann-Whitney.

To test the normality, what value should I use? Is ct or dct (after normalization ct)?
The residuals of your model - and that model should be fitted using the dCt values.
