We have all had the odd article that struggles to find a home. However, sometimes you come across articles that are just so 'bad', that you doubt whether any journal will accept them.
What is the most number of rejections you have seen for a single article?
Did the author make efforts to address any issues that were previously raised?
What do you do if you realise it is the research behind the article which is at fault (not issues such as language, journal fit etc.)?
How do you get authors to honestly evaluate their papers before submission?
Well, very interesting and painful question..hehe. Actually if a paper is being rejected multiple times, that doesn't necessarily mean that the paper quality is bad, it could just be case of choosing wrong journal or may be wrong reviewers. I have paper that is bouncing between journals editors reviewers since 2 and half years, but still under process. Rejected multiple times, but every time I feel the reviewer has fail to properly understand my point of view. And yes I try to address the reviewers comments each time and try to enhance the quality of technical writing, and explanation. For your third question, that is a case that always lingers, but luckily reviewers don't seem to point any such thing in my case. However, the flaw in research must be highlighted and pointed out in early stages of research study period. I believe this is where your supervisor comes into play. It's the prime responsibility of supervisor to guide and ensure that the research students - working under their supervision, are on the right track and doing research in correct manner and on correct significantly important topic.
I think for your 4th question, it's difficult to truly self analyze your research contribution. Usually all the researchers I have talked to and worked with, feel at some point that their research work and contribution is not of significant worth. Ironic that is. But I believe again, here, it's your supervisor prime responsibility to guide and evaluate your work critically, before acknowledging for further process.

Did the author make efforts to address any issues that were previously raised? Yes and the advice was given to try a journal that was more suitable
What do you do if you realise it is the research behind the article which is at fault (not issues such as language, journal fit etc.)? Never had the experience, but I would definitely revisit the methodology
How do you get authors to honestly evaluate their papers before submission? By double checking the criteria for submission as published by the Journal against the article
