It is an intriguing question. Even though exact age can not be given, Ricketts has discussed about treatment as early as in deciduous dentition in his Bioprogressive therapy.
as soon as 6 year molars finish their apex closure some where around 7 -8 years of agethey are ready for funcional appliance which not only will jump anterior cross bite ,but eill develop arches and midface.My favorite appliance is 3 way saggital with a face mask.Require patient compliance which is not difficult to achieve at this age
it can be treated at any age group but treatment modalities differs agewise..during growth we can use myofunctional or headgear..after growth lapses we can opt for surgical orthodontics to correct cl3 malocclusion..
The RCT by Mandall et al., stated that it should be done before 10 years of age. I have tried the YC3 allpliance for a 4 year old boy and it worked. Does the use of Expansion appliance and face mask if it is used at early age causes facial asymetry?
In my opinion it depends on the amount of discrepancy, the presence or not of skeletal problem associated and the presence or not of crossbite.
If the patient shows an accentuated discrepancy, with crossbite and skeletal problem associated, I think you have to treat as soon as possible, even in deciduous dentition, to allow an harmonic development.
If the discrepancy is slight or moderate, without crossbite, you may wait until the late mixed dentition, to treat only in one phase.
I would like to wait for the eruption of the upper front teeth and then start the treatment. However Prof. Hugo De clerk has suggested to strat the treatment with class III later (up 9-10 years) the the bone quality is better and the relapse after the treatment is less than the early treatment.
