An effective meeting serves a useful purpose. This means that in it, you achieve a desired outcome. For a meeting to meet this outcome, or objective, you have to be clear about what it is. Too often, people call a meeting to discuss something without really considering what a good outcome would be. And a bad meeting is the meeting that is unplanned and not stick to the agenda.
When meeting is in progressive line & its agenda is known to the member attending the meeting .It is likely that the meeting may offer the positive result in the line with all the discussion covered in the meeting . In bad meeting sometime unhealthy discussion without coming out under the positive manner .
In my opinion, the difference between a good and bad meeting is the outcome of the meeting, but whether the results are negative or positive, It is useful to you and you learn from it..
A good meeting is the one in which there is a sense of intention ,sense of direction,sense of learning , sense of individuality and uniqueness,sense of valuing innovative ideas,sense of psychological phenomena like groupthink and sense of promoting motivation and collaboration.

1: A good meeting is well pre-prepared with a clear agenda including what to be discussed during the meeting, whereas a bad one is non prepared with no clear agenda & non identified topics to be included.
2: A good meeting ends up with a clear plan of action to be implemented & the reverse could be said about the bad one.
3: A good meeting include the input from all participants while the bad one does not include sharing of all opinions & feed back.
A good meeting is a meeting that ends with every one being tasked and unhappy while a bad one is that which ends with everyone laughing.........Deep definition...Lols
From my experience, a bad meeting is where a problem has occurred but nobody wants to talk about the cause and people are proposing solutions to serve the whims of the executives in charge. Now there's also an ugly meeting whereas people are living in lies and trying to manipulate others in believing the untrue facts just for the sake of conning the customers or putting on a magic show to get their decisions approved by the committee.

Having a good reason to meet ,Having an agenda, a time-frame , few exceptions; well prepared participants, a facilitator who can keep participants focused on the agenda items.
Thank you for your thought-provoking addition to this discussion. It is true that many meeting is 1-way communication and no feedback. The free exchange of information is usually ignored in a meeting. This does not understand the needs and views of all people.
Clearly defined agenda is important for a good meeting.Agenda needs to be circulated well in advance. Participants need to share their views freely. The meeting need to define actionable points with the responsibility defined.
In a good meeting somebody takes ownership of recommendations and follows through with implementation - which normally requires a few more good meetings.
After a bad meeting everybody quickly ignores the information provided at the meeting and continues business as usual and no progress is made.
