if size 6 mm use elliptical incision with fine suturing

Medical therapy to reduce final scars for 2-3 month; all this will help in your clinical practice.

That depends from it's clinical appearance: size (see answer above) and dermoscopic findings.

If there is any suspicion, then excision with histologic examination is mandatory. Fine suturing, intracutaneous wherever it is useful. If you are dealing with large lentigos, try laser as described by Mohd Asif above.

Nd/YAG Q switch laser

For intradermal naevi then radiosurgery shaving (Ellman) does a great job.

If they are superficial clearly benign (freckles etc) and in younger patients, I shave them in an intradermal plane.- and let the wound epithelialize from the hair follicles. There is a 20 % chnce of recurrence, however, but young people bear the same chance of a hypertrophic or hyperpigmented scar !

Ellman-radio-shaving will do the same.  but if you use magnifying glasses, you can see in surgery possible melanin-nests and go deeper.

Of course, first it has to be great surgery and then the best way to make scars less visible is to treat the skin with vitamin A before the operation and then to do skin needling as soon after the surgery as is comfortable.  I start 2 days later and repeat a number of times every 5 to 7 days and the TGF-beta3 and IL 10 growth factors ensure regeneration and even scarless healing as Fergusson intended.

Shaving (with a surgical blade)with secondary healing is a good option .Post-op avoidance of sun light plus other scar prevention maneuvers are to be taken care of ;results are excellent with us. if large ones,results are still good with primary closure with proper wound/scar management. Lasers can also be tried.
