1. A 1-item scale from Spector, Dwyer, & Jex 1988
How often have your seriously considered quitting your current job?
Never Somewhat often
Rarely Quite often
Sometimes Extremely often
2. The 3-item intention scale from the Michigan Organizational Assessment Questionnaire (MOAQ) by Cammann et al.
Maybe you could take a look at the Turnover Intention Scale (TIS-6). I haven't been able to figure out if the questionnaire is reliable, but I'm sure research has the answer for you.

I have a 2-item scale for employee turnover intention in the attached publication (see page 3).
I have had success using the following scale:
1. I often think about quitting this organization.
2. I intend to search for a position with another employer in the next year.
