Histopathological examination of GCTs reveals several characteristic features, the most notable being the presence of Call-Exner bodies—small, rosette-like structures formed by granulosa cells. Additionally, the nuclei of granulosa cells often have a "coffee bean" appearance due to longitudinal nuclear grooves. Immunohistochemical staining for markers such as inhibin, calretinin, and FOXL2 (particularly the FOXL2 402C>G mutation in AGCTs) further aids in diagnosis by confirming the sex cord-stromal origin of the tumor.
