Nice question. First we need to understand if we should take it up as a software engineering project!! In my view it should be!! others may differ. There are many ways. Having said that I would be eager, if I was in that position, to pursue SDLC with user centered design approach. While doing so, heath policy and infrastructure should also be studied in detail. As regards indicators, there are many globally as well as each nation would have portfolios!!! One could take them up for bench marking and localizing them for broader benefits.

Align your project according to the SDLC.
- Identify the Business process and business functionalities
- Do a feasibility study to get an idea about the infrastructure
- Study similar systems and or existing system
- Based on the above studies create a mind map
- According to the mind map, Identify and verify objectives and scope
- Cross check again whether the scope and objectives are clear or not
- Develop design documents focusing SDLC
