Good water source especially for drinking is a serious challenge in developing countries , couple with poor regulation by watch dog institutions in hygiene for potable water supply , Widal test is still positive for both clinical and non clinical condition can this issue be mitigated through efficient and otherwise other rapid test methods.
I can say firmly that Widal test have very important role in the management of typhoid fever in India. Typhoid is a very common disease in India both in rural and urban areas. The disease is easily detected by Widal test.
Agree with Arvind. Vidal test is very common in India for diagnosing typhoid and its intensity. It is necessary to have some cross checking. Because based on Vidal test a long term medicinal course started.
In Sri Lanka, we try to diagnose with blood cultures but the prior antibiotic therapy at the out-patient setting or at the general practitioner level is the biggest problem. The clinicians often use widal in culture negative or in culture-not possible settings but the negative test should be interpreted with caution. The clinical presentation can be mixed up with Dengue or leptospirosis or even with typhus perhaps so the correct diagnosis is really important in a country where all such infections exist. Therefore widal can be considered as a useful tool in certain circumstances.
