What are the role of Sex Hormone Binding Globulin (SHBG) in Liver cirrhosis ?
Is it increased or decreased and why ?
As part of the increase in globulin levels in cirrhosis, SHBG is also increased which binds testosterone leading to hypogonadism in cirrhotic males, especially in alcohol related cirrhosis. However lipogenesis decrease SHBG levels and hence low levels are found in NAFLD related cirrhosis. In fact higher SHBG levels protect against NAFLD. There is also changed SHBG isoforms with different steroid-binding affinities.
In female oestrogen increases fatty liver, therefore low level of SHBG increases free oestrogen level since considered as risk of liver cirrhosis.
