Recent guidlines emphasized the importance of cognitive-behavioral therapy and multimodal treatment: pharmaceutical ( Amitriptyline, Pregabalin and Duloxetine ..) and non pharmaceutical such as physical therapy. It is crucial that treatment aproach remains individualized and talored for each patient.
People with fibromyalgia also have "tender points" throughout their bodies . The body has 18 sites that are possible tender points.Cognitive, memory problems (sometimes called "fibro fog", and Trouble sleeping are main possible signs. People with certain other diseases, such as rheumatoid arthritis, lupus, and other types of arthritis, may be more likely to have it. Treatment often requires a team approach . Pain , sleep treatment , psychological support , Biofeedback and Osteopathic or chiropractic manipulation are main stay of treatment
We have the hypothesis that the fibromialgic syndrome is related to a generalized EDEMA IN THE DEEP SUBCUTANEOUS FATTY TISSUE. This edema causes a painful distension that in certain critic anatomic areas correspond with the “paindul spots”.
IF the patient is palpated carefully and with LOTION on the fingers, this edema can be felt like deep subcutanous NODULES In certain parts.
This theory was supported already by Copeman in 1949
This is one of his papers. Free on line. 1: COPEMAN WS. Fibro-fatty tissue and its relation to certain rheumaticsyndromes. Ann R Coll Surg Engl. 1949 Aug;5(2):110-29. PubMed PMID: 18134452; PubMed Central PMCID: PMC2238360.
The distension would cause pain and even compression of the peripheric nerves.
We are actually studying this theory.
We think that fatty tissue has been neglected as a source of pain. And very little is known of its capacity of creating edema (Under certain circumstances). Once we studied better the fatty tissue many UNEXPLAIN PAINS will be more explainable.
The encapsulated edema of the deep fatty tissue can be palpated as nodules, bands, hardenings or painful spots. The accumulation tension can produce peripheral neuropathy by compression.
I will attach to you the photo as how they can look from a surgery image.
