DM is known to longer the QT intervals and there is evidence that NAFL also longers the QT intervals. what are the other effects and possible mechanisms?
Non-alcoholic fatty liver disease (NAFLD) has been shown to be arrhythmogenic from years of research. Some possible mechanisms include the autonomic dysfunction due to the inflammatory milieu of NAFLD. My doctoral dissertation was on inflammation and I studied many of the potential consequences of inflammation on the heart. I have included some studies which demonstrate a connection not only between NAFLD and atrial fibrillation (Targher et al., 2014), but also other arrhythmias (Ballestri et al., 2013). Non-alcoholic fatty liver disease (NAFLD) is caused by oxidative dehydration induced stress (OD-S). The OD-S increases the risk of developing cardiovascular disease. The metabolic processes underlying NAFLD is part of an integrated mechanism that may differentially affect the heart. As such the alteration will show up in the Exercise Tolerance Test (ETT) or Stress ECG.

NAFLD is actually part of the metabolic syndrome to which also belongs ischemic heart disease and DM. This is also the reason why NAFLD has been shown to increase risk of cardiovascular disease. The pathophysiologic mechanism of metabolic syndrome [hypertension, hyperlipidemia, insulin resistance] also underlie the pathogenesis of NAFLD and heart disease like IHD, arrhythmia etc.
