There are several. Which of those is most important will depend on sector and specific business, I guess. Just listing a few:
First, there are trade-offs. Between environmental and economical aspects (e.g. more sustainable material, but more expensive), but also between different environmental (or social) aspects (e.g. less/non toxic material, but requires more of it, or higher embodied energy).
Second, there are typical business consideration related to innovation: lock-in effects of major investments / switching to new (at least new to firm) technology and partners / becoming reliant on single supplier of sustainable material etc.
Third, there are internal organisational issues. Overcoming resistance to change in your organisation.
Fourth, external factors. Legislation may prohibit the best solution or force a focus on aspects that are actually of lesser concern for your industry/product. Other partners in the value chain may have more power than you and resist or push for alternative directions.
