I'm using Ryff's Psychological Well-Being Scale, as well as Waterman's Questionnaire for Eudaimonic Well Being (QEWB). Those seem to be the two major tools.
Corey Keys distinguished between psychological and social elements of eudaimonia, making it a potentially comprehensive measure. He built on the work of Ryff. These two elements of eudaimonia, along with hedonia are measured by the Mental Health Continuum. It is also available in a short form.

I have studied eudaimonic well-being partially in my thesis, and think that Ryff's scale is an adequate choice. Mental Health Continuum – Short Form by Keyes is also devised for eudaimonic well-being.
