We have many techniques for revascularization [angiplasties, new stents, vascular bypass], yet we are not doing early screening in patients with multiple vascular risk factors.
Should we be more active in our early screening of the carotid and cerebral vessel circulations?
Are we missing the boat?
I think more attention should be given to screening which excludes AD cases, as many patients we studied were mistakenly included in the AD group by non neurological previous examination, consequently the vascular dementia diagnose came rather late. GP should encourage patients and family members to get accurate neurological check as early as first memory and confabulation signs appear.
I think that angioplasty, bypass, stenting are of value for large vessel diseases astherosclerosis and subsequent thrombosis which if happened will cause stroke that may or may not be followed by vascular dementia..
Yet the aforementioned techniques are not valid for small vessel disease which has a good share in vascular dementia..
I think proper screening for modifiable vascular risk factors comes first for better primary prevention which in turn will protect against small vessel as well as large vessel disease ..
Beside annual screening for cognitive functions that will detect subtle signs and keep an active cognitive brain through cognitive enhancing exercises as well.
