This is the largest meta-analysis including critical events and mortality data on patients prescribed ACEi/ARB and found evidence of beneficial effects of chronic ACEi/ARB use especially in hypertensive cohort with COVID-19. As such, we would strongly encourage patients to continue with RAAS inhibitor pharmacotherapy during the COVID-19 pandemic.

Covid 19 virus appears to enter into human cells through the ACE2 receptor so there are relevant theoretical concerns that drugs which influence ACE2 cell surface receptor expression (eg, renin–angiotensin system antagonists) may increase susceptibility to infection and to the severity of the illness.

Using retrospective electronic health data from a large US health system, NYU Langone Health, these investigators found that, among patients tested for COVID-19, hypertension was common (35%) and associated with high rates of positive COVID status (59%) and severe illness (25%). However, the hypertensive drug class (RAS antagonists, beta blockers, calcium channel blockers, and thiazide diuretics) was not associated with the likelihood of a positive test or risk of severe illness. These data help to confirm the very early recommendations from major cardiovascular societies (AHA, ACC, HFSA) that, despite the biologic mechanisms underlying COVID-19 pathogenesis, patients should continue their use of their antihypertensive agents, including ACEi and ARBs.

COVID-19 influence the expression of ACE2 cell serfece receptor, it could lead to more formation of angiotensin-2, which causes vasoconstriction and enhances aldosterone, furthermore aldosterone causes retention of sodium and hypertension.
