Simple decompressive laminectomies is usually adequate and optimal care. Fusion will be necessary if there is instability and or loss of cervical lordosis. The choice of laminoplasty depends on individual surgeon.

straight neck/ cervical kyphosis= laminectomy+fusion

lordosis preserved = laminoplasty

Axial neck pain is also very important. In the presence of severe neck pain fusion will be a better choice.

It depends on the etiology of myelopathy. If simple DJD sometimes laminectomy alone is sufficient. If the cause is OPLL you need anterior approach sometimes followed by laminectomy and instrumentation. Laminoplasty is of use in children and adolescents as neck musculature is strong enough in adults to ignore laminae.
