yes chlorexidine disinfection of superficial skin area is advisable before using a penetrating skin cream, in case of risk of infection due to the working area.
I developpped the Katoss formula: an air spray to clean the air of surgery room. Tested oon H1N1 virus but at distance as it is irritating : a spray pump which is plugged outside the surgical area, for 15 minutes: easy and cheap.

My personal opinion, I think that any solution including ultrasound gel might be neurotoxic in certain individuals. Depends on many factors.
But if you comply with recommendations (let it dry before puncture, contamination of gloves...) there is only a little evidence to support claims about significant risk. Depends on idividual doctor scrupulous attitude.

I have a formula for penetrating water into the skin , dosed more than 95% in an emulsion (special process) : it should be interested to add 0,5 to 1% of Chlorexiidine in case of severe pain/ infection. Just a test : it could be used toward deeper desinfection. for ex in fingers area .( Panari). Maybe. We would reduce volatility of alcohol base. Just an idea...
