COVID-19 related fatalities and pathological autopsy in medical centres have revealed thrombosis phenomenon extensively in lungs and kidneys. Considering fulminant and widespread thrombosis manifestation in the disease, it may be safer to add anticoagulant as routine protocol in any surgical intervention to reduce the risk of thrombosis. This practice may be incorporated as a protocol for safer surgery to reduce complications.

Can we manage prophylactic therapy in COVID-19 patients to prevent severe illness complications?
Many patients with COVID-19 have thromboembolic complications that worsen their prognosis. Herein, the authors propose a modified version of the CHA 2 DS 2-VASc score, including 1 point for COVID-19, so that prophylaxis to protect against thromboembolic events would be indicated before the condition becomes severe. The advantages of this modification would be prevention of the patient' s condition worsening due to thromboembolic problems and reduction of the likelihood of a need for intensive care and mechanical ventilation, reducing mortality.
Yes, any COVID-19 documented patient with critical illness should be on anticoagulation preferably with low molecular heparin unless there is a contraindication.

I think the patients should be assessed with hydration, hematocrit, erythrocytes sedimentation rate, serum ferritin, and D-dimer to select for using anticoagulants. Please see our experience here:

No, there is enough data from surgeries carried out during COVID period that shows no increase mortality from respiratory and cardiac events specially that due to coagulopathies. Most data is on covid negative patients but there is data on covid positive surgeries as well. Recommendation for anticoagulant remains as before
