I am working on a tool to deliver stand-alone online CBT for depressive symptoms. In this tool, we are assessing the user's depressive symptoms using PHQ-9 questionnaire. We have seen that the 9th question in PHQ-9 has a very high false positive rate assessing suicidal ideation. This is because the 9th question assesses both passive death wishes and active suicidal thoughts. So, if a user shows any suicidal ideation on the 9th question in PHQ-9, we are planning to probe the user a little bit more to ascertain that the user actually has active suicidal thoughts before raising an alarm. So, I am searching for a self-administered questionnaire to assess suicidal ideation. Can anyone please suggest such a questionnaire?

All the questionnaires that I have come across - C-SSRS, BSSI, etc. - are supposed to be administered by professionals. Thanks in advance.
