Dear Dr. Gupta, I have a patient with disseminated sarcoidosis of lung bone, spleen, lymph nodes and I am wondering if he has an occult infection, such as MTB, atypical mycobacteria, actinomycosis, or fungal disease. No organisms seen on LN biopsy. I am considering using a bacterial DNA test from the company IDbyDNA, Inc. for further workup.

I came across your trial on NCI.gov and was wondering if you have any data to support the presumed connection between occult infection and TB. He also has cholangiocarcinoma and early stage prostate cancer, and we are unable to use PET scanning to follow his malignancies because the sarcoid lesions are hyper metabolic. I also worry that the inflammatory milieu in his body may provoke or stimulate any malignant cells that are resident in the body.
