The framing of questionnaire depends on the intentions of research and mode of collection of data. The questionnaire should mainly contain personal data and also should show the difference in gender when it comes to awareness of organic food.

The questionnaire should show the difference in gender and age concern. Also facts like animal welfare, the environment, or preferences in taste. But for sure the economic facts are very important as well and ahould be considered.
if you may distinguish per sector (like fresh fruits and vegetables) it is important to add some questions about the aesthetic aspects, because it may influence on consumer decisions
