I’m preparing a questionnaire (or interviews?) for designing a definition of specific term (I don’t want to give the “name” of this term). This term is often used in scientific literature, but no one (I didn’t found any) has defined it (what is the meaning of this term). I’m searching for some sample of this kind of questionnaire? Can anyone help me with some sample of questionnaire which was designed for the same purpose (to prepare a definition)?

I would actually start with a qualitative process - a Delphi process would probably be the norm in this kind of situation. You can get experts together to see if they can agree on a definition or definitions and then test them quantitatively.

For the term you want to define, Probably listing various scopes or meanings on scalable response bar (as part of the survey) will help you to get experts meaning preferences and then may be you want to try formulate the definition based on the frequency or commonality of responses as long as you limit it to the field experts.
I agree with Ronan that qualitative methods are often used to develop the content for a questionnaire. In addition to Delphi methods, you might consider focus groups with people who are familiar with the concept in question, or key informant interviews with those who are highly involved or especially knowledgable.
