A small public health screening study based Questionnaire for CKD and ESRD. From this questionnaire, we should extract screening data, prevalence and incidence info, economic-based treatment data & quality of treatment and recommended risk factors. We would like to have every aids to do a comprehensive questionnaire.

Therefore, the detection and progression of renal disease depend more on values such as serum creatinine concentration, creatinine clearance or glomerular filtration rate (GFR), stage of renal insufficiency, protein excretion, and complications such as hyperphosphatemia, anemia, or hypertension.
You are right Dr. Josef. We aims to have a comprehensive questionnaire as a screening tool for CKD and ESRD in a population-based study. We will use Demographic data of these patients along with eGFR values, serum and urinary Albumin, serum creatinine, serum Cystatin C if applicable, serum phosphorus and calcium levels too. As I told we need to have a new structured questionnaire to highlight too on Dialysis , treatment quality and the economic burden too.

Be sure that, beyond biological data (creatinine, haemoglobin, phosphorus, calcium, potation, bicarbonate, albuminuria, proteinuria, etc,...) you also consider screening for the experience of living with CKD, which is often mismatched with CKD staging. For that I suggest, not only quality of life scales (KDQoL or EQ-5D), but also functionality (SPPB, for example) and disability scales (WHODAS, for example). You may find interesting points and predictors.
Also the concept of Satisfaction With Life could be explored, but there you would have to correlate with indices of depression (BDI, for exemple) and other psychometric scales. They're endless.
