Maybe somebody has some information, some tips or some examples how a questionnaire like this should be made? I am a medical student and I would like to try to do some research about Knowledge of EC among the medical and non-medical students and compare the results. My problem is that I am not really sure how a questionnaire like this should be structured.

We have devised our own six item knowledge questionnaire. Each item was a statement (e.g. “Emergency contraception only works when a woman is not (yet) pregnant”) with three possible responses: “correct”, “incorrect” and “don’t know”. The items did not form a scale in our study (α=.49). If you need the other items, I'll translate them for you ([email protected]).
www.surveymonkey.com is great for small surveys. Anything under 10 questions is free and they have reasonable subscription rates for larger surveys. If you notice, George posted a surveymonkey survey. I, however, would not complete the survey George posted because it is all essay/narrative formatted questions. I am too busy to commit to writing out nine short answer style responses. Due to short online attention spans and an assumed lack of interest in completing a survey, multiple choice or checkbox type questions are likely to get you a lot more participation. As a bonus, you can create choice matrixes in surveymonkey effectively turning a single question into 10-12 data collection points, thus getting much more info under the 10 question free plan. Surveymonkey also allows unique IP address tracking, specific link generation, and a host of other stat tools. Some of these tools are included in the free plan and others are not. I think you have to get a paid account in order to export to Excel or SPSS.
George’s survey gives a very good example of inserting informed consent into an online survey.
