What type of public health program are you considering establishing? Would it have an educational focus, infectious disease, chronic disease, behavioral health, regulatory, or ???
I set up a program in an underserved area to ensure healthy participants. An RN did an assessment, health advocates (high school educated) implemented the treatment plans which included transportation allocation to medical visits, notes to Primary Care Providers for recommended preventative care (once ordered the health advocates helped people complete them i.e. mammograms, blood work, colorectal cancer screening, etc.). They took people to classes in the community on diabetes, etc. If interested contact me. I did the statistics and we had minimal care, but in the end, found people with stage 4 cancer (got hospice involved), successful completion of preventative tests (about 10 each), coordination of care between specialists and primary care providers, etc. If interested email me at [email protected].
