If you want to conduct study on patients then ethical clearance from institute/hospital is mandatory prior to study and second thing is that patient consent should be taken.general information we can collect from patient but it's ethical wrong to circulate patients documents data in anyway.

Your state and medical facilities should have specific rules and regulations about reviewing patient records. The ANA may have information on steps to take when needing data.
