Its easy for me to assume that the sequence of treatment should be Apexification, restoration (Crown) and then orthodontic movement. However, I would like to have a lot more information about the status of the tooth prior to taking a decision. For a central incisor to have open apices even if the trauma happened 5 years ago (when she was 11 years and assumably the root apex should have been closed by then) I would truly like to know more - tooth mobility status, level of tooth fracture, level of bone support, health status of the child.  

Thank you Dr Folyan. Even I was surprised to know that tooth had trauma at the age of 11 years and still apex was open. I confirmed the history twice with her mother.

Currently only around 3 mm anatomical crown structure is present, on the labial aspect and 2 mm on palatal aspect. Tooth is asymptomatic. There is no mobility while periapical radiograph shows only 1 mm horizontal bone loss. Child is otherwise healthy and is maintaining good oral hygiene. 

I agree with the recommendation of Dr. Folyan, i.e., apexification, followed by restoration, then orthodontic treatment.  The considerations Dr. Folyan mentions are important to understand, as would be periodic observation of the root length status throughout the active phase of orthodontic mechanotherapy.

I might suggest another treatment plan depending on tooth status (3 mm tooth structure and open apex):

The suggestions of Morenike Oluwatoyin Folayan are valuable if it is a non-extraction orthodontic treatment, however, Ahmed Elkhadem's alternative plan is sensible, if it is an extraction case (four premolars).

If the nonvital immature incisor has to be retained, regenerative procedures (pulp revascularization, PRF etc.) should be chosen ahead of simple apexification with MTA. 

I agree with the treatment proposed by Dr. Folayan, although we know that it's possible to move devitalized tooth after rct done  without get root resorption in this specific case presented by Dr.Ahad as the tooth already present root length shorter compared with the adjacent tooth and pre traumatized tooth is a risk factor for root resorption, during orthodontic movement you might get root resorption even thought the apex still open. During orthodontic phase of your comprehensive dental treatment, you have to observe periodically the root length of the incisors by taking PA's and if  you observe that some sort of root resorption start to develop you have to stop the orthodontic tooth movement for a period of time (usually 3 months) and observe, if the resorption persist you have to resume the orthodontic treatment.

The option that was given by Dr. Elkhadem is good although we have to observe the pros and cons before go for this sort of treatment, i am talking about the ankylosis after replantation, shape of the premolars to match with the central (root length/M-D/B-L widths of the root and so on), position of the smile line that can mask any discrepancy between the gingival margin that might be created, aesthetics is detrimental in that region. 

The main point here is, despite the options and treatments given in that debate, the patient should be aware of all possibilities and compromises that can happen during and after the treatment done.  Long term follow up is essential for this patient.

Apexification and any other endodontic, periodontal, and restorative work need to completed before orthodontic treatment.
