I just happened to find your questionnare on RG and clicked on it out of curiosity. Please make sure to include information according to the General Data Protection Regulation by the EU before participants can access your questionnaire. Since it is anonymous, the amount of information you need to provide is very limited but goes beyond the scope of what you have explained here and it is preferable to have it included directly in the survey anyway. It is a complicated issues so I recommend checking with your university's data protection officer.
