There quite many Public health issues that need to be given attention but if you are looking for a PhD topic is better to focus in your area of specialty. other your likely to get as many as possible beyond your scope.

Titus - you would need to be a little more specific. Public health is a varied and huge discipline. For a PhD - as it is a longitudinal commitment - it is advisable that you choose a topic that is related to your current interests and, perhaps, previous studies/research that you have completed.
Look around your area of specialty and your passion as stated earlier. Look at recommendations made in previous studies that can guide you on the gaps. Use that as your starting point. You need to have interest and passion in what you want to do, otherwise the lights may go off in the process.
